| 1. | Name | Inquiries about the name which would indicate the applicant's lineage, ancestry, national origin, descent or marital status. Inquiry into previous name of applicant where it has been changed by court order or otherwise. |
| 2. | Marital and family Status | Any inquiry indicating whether an applicant is married, single, divorced, engaged, etc. Number and age of children. Information on child-care arrangements. Any questions concerning pregnancy. |
| 3. | Age | Request that applicant state age or date of birth. |
| 4. | Handicaps | General inquiries (e.g., "Do you have any handicaps?") which would require applicant to divulge handicaps or health conditions which do not relate reasonably to fitness to perform job. |
| 5. | Spouse | Questions related to spouse. |
| 6. | Race or Color | Questions directly or indirectly indicating race or color. |
| 7. | Address or Duration of Residence | Names or relationship of persons with whom applicant resides. Whether applicant owns or rents home. |
| 8. | Birthplace | Any inquiry into national origin. |
| 9. | Religion | Applicant's religious denomination or affiliation, church, parish, pastor, or religious holidays observed. |
| 10. | Military Record | Type of discharge from the service. |
| 11. | Citizenship | Whether applicant or his/her parents or spouse are naturalized or native-born U.S. citizens. |
| 12. | Ancestry or National Origin | Inquiries into applicant's lineage, ancestry, national origin, descent, birthplace, or mother tongue. National origin of applicant's parents or spouse. |
| 13. | Education | Inquiry as to what is the applicant's mother tongue or how foreign language ability was acquired. |
| 14. | Conviction, Arrest or Court Record | Any inquiry relating to arrests. |
| 15. | Relatives | With whom do you reside? Do you live with your parents? |
| 16. | Organizations | The names of organizations to which the applicant belongs if such information would indicate through character or name the race, religion, color or ancestry of the membership. |
